Welcome to the Dennis Yarmouth Middle School Nurse's Page. This is where I will be periodically posting important health information, dates of upcoming events, and other tidbits to keep your student happy, healthy, and in school. Read below for some of the health related items that maybe important to the well being of your child.

Immunization Requirements

By the start of grade 7, all students are required to have an Updated
physical exam and be in compliance with the Massachusetts School
Immunization Requirements:
Immunizations needed before grade 7 are a Tdap booster, and a second Varicella vaccine. Starting 2020-2021 school year the Meningococcal Conjugate Vaccine is also required.
As soon as these requirements are fulfilled, please give a copy of the
paperwork to the School Nurse!

couple of free clinic options on Cape Cod.

Be ready for Fall sports this school year. You need a physical that is within the last 13 months to participate in sports.

If your child needs to take medication during the school day, there are
some important Massachusetts regulations that must be followed.
  • A medication order from a Massachusetts licensed medical provider
  • must be on file with the school nurse. Medication Order
  • Written parent/guardian consent to administer medications at school
  • must be on file with the school nurse. Parent/guardian consent for
  • medication at school.
  • This medication must be brought in to school by a responsible adult.
  • School children are NOT to bring ANY medications to school.
  • This medication must be in the original prescription bottle with the
  • student's name and MD order on the label. If an over the counter
  • medication is prescribed, it must be in the original medication bottle.
